The Guarantor's Name can be found on the 1st (first) page of your Home Health bill, on the left side of the page, above the dotted line. It will be on the first line of the address

Select Bill Type
Your Account Number can be found on the 1st (first) page of your Home Health bill, on the right side of the page, on the last line of the Credit Card box, under the ACCOUNT NUMBER label.
Your Account Number will begin with HH or J followed by several digits

Find Zip Code
The Guarantor's Zip Code can be found on the 1st (first) page of your Home Health bill, on the left side of the page, just above the dotted line. It is the 5 digit number located at the end of the third line of the address
