The Guarantor's Name can be found on the 1st (first) page of your Physician's Network bill, on the left side of the page, near the top of the page. It will be the first line of the address and is in the format: Last, First

Select Bill Type
Your Account Number can be found on the 1st (first) page of your Physician's Network bill, on the right side of the page, near the top of the page. It can be found in the Credit Card section under the heading: ACCT. #
The Statement Number can be found on the 1st (first) page of your Physician's Network bill, on the right side of the page, near the top of the page. It can be found in the Credit Card section under the heading: STATEMENT #

Find Zip Code
The Guarantor's Zip Code can be found on the 1st (first) page of your Physician's Network bill, on left side of the page, near the top of the page. It is the 5 digit number located at the end of the third line of the address
