Find Guarantor's Last Name
The Guarantor's Name can be found on the 1st (first) page of your Quick Care bill, on the left side of the page, near the top of the page. It will be the first line of the address and is in the format: Last, First Middle

Find Account / Statement #
Select Bill Type
Your Account Number can be found on the 2nd (second) page of your Quick Care bill, on the right side of the page. It will be the sequence of letters and numbers after the Account Number: label.
Your Quick Care bill may contain more than one Account Number
The Statement Number can be found on the 1st (first) page of your Quick Care bill, on the left side of the page, near the top of the page. It will be the sequence of letters and numbers following the 'Statement Number:' label

Find Zip Code
The Guarantor's Zip Code can be found on the 1st (first) page of your Quick Care bill, on the left side of the page, near the top of the page. It is the 5 digit number located at the end of the third line of the address
